Please fill out this form completely. When you finish click on the "Enter Payment Information"
button and you will be redirected to the Payment Page.
Once you have completed online payment, your child will be registered for this activity.

Please note that new families will need to fill out our Parental Consent Form
and provide a copy of each child's Baptism Certificate (if not baptized at St. Francis of Assisi)
before they can begin attending classes. Your confirmation email will provide a link to directly upload those documents to us.

Note: If you have given us a Baptism Certificate in a prior year you do not need to turn in another one.
